• Insert the Username: AHNS (all caps) and Password: expo0719 (lowercase)
• Please note the username and password are case sensitive
• The Show Summary Information will be displayed as an easy reference
• Scroll down to "Download Curtin ESK" and print the document
• Then, return to the Order Page, and you will find the Important Association Forms Packet to download.
• The forms are all using Adobe Reader, and it is available to install on the website.
• All forms need to be printed out and completed via hard copy. They are not interactive. Once you have completed the forms, please fax the Curtin forms to Curtin (415)883-1755. All other order forms and payments should be faxed directly to the appropriate company.
