  • NOTE: The session chair/lead is the person(s) responsible for organizing the session. This may or may not be the session moderator. Please list the session moderator/anyone making an introduction in the Session Outline grid towards the bottom of the form.

If the session is more of a panel discussion vs individual talks, please include the cases to be discussed if possible and in the order you plan to discuss them. If you do not know the exact cases/questions you will be discussing, you can add them as "case 1", "question 1", etc.
