FOR ALL PROGRAMS
Program Change Request Form
Any significant changes to your fellowship program (i.e. Program Chair change, addition or deletion of fellow positions, significant curricular or structural changes) must be reviewed and approved by the AHNS Advanced Training Council – use this form to submit those requests.
Trainee Evaluation Form – To be completed by each fellow’s training program
Program directors must evaluate the clinical and technical competence of trainees on a regular periodic basis (twice a year, at the mid-point and end of fellowship is the required minimum) as stated in the ATC program guidelines. The fellow’s evaluation documentation must be provided to the ATC as part of the fellow’s final assessment and satisfactory completion of the fellowship’s training requirements.
Fellowship Interview Calendar – AHNS Accredited programs are invited to list their fellowship interview dates on this calendar – they are not required to list them, and names of interview candidates will not appear.
