Biography/Statement

Dr. Bearelly is originally from San Jose, California. He attended college and medical school at Boston University. He then completed a 5-year residency training program in Otolaryngology – Head and Neck Surgery at the University of California, San Francisco. Following residency, he completed a fellowship in Head and Neck Surgical Oncology and Microvascular Reconstruction at Vanderbilt University Medical Center. Dr. Bearelly makes a significant effort to try and explain the disease process and treatment plan to his patients and their families. He individualizes and customizes the treatment because he knows every patient is different. When he is away from work, he enjoys the outdoors and spending time with his family.

Biography/Statement

Wojtek Mydlarz, M.D., is the director of Johns Hopkins Head and Neck Surgery for the National Capitol Region, as well as an assistant professor in the Department of Otolaryngology–Head and Neck Surgery for Johns Hopkins University School of Medicine. Dr. Mydlarz is board certified in Otolaryngology and specializes in the treatment of benign and malignant tumors of the head and neck, including the upper aerodigestive tract, salivary glands, thyroid, skin and soft tissue.

Dr. Mydlarz received his medical degree from The Johns Hopkins University School of Medicine and completed his internship and residency in Otolaryngology – Head and Neck Surgery at Johns Hopkins Hospital. He received specialized head and neck cancer research training fellowships sponsored by the National Institutes of Health. He also completed a Science of Clinical Investigation Training Program at Johns Hopkins Bloomberg School of Public Health, as well as specialized fellowship training in head and neck surgical oncology at M.D. Anderson Cancer Center in Houston.

Dr. Mydlarz was awarded the Helen Taussig Professionalism and Career Development Award given out by the Johns Hopkins University School of Medicine. He was inducted into the Alpha Omega Alpha Honor Medical Society.

Professionally he is an active member of the American Head and Neck Society, where he serves on various committees. He is also a fellow of the American Academy of Otolaryngology – Head and Neck Surgery, as well as the American College of Surgeons.

Dr. Mydlarz is a medical officer, as well as a clinical and research collaborator in the Head and Neck Surgery Branch of the National Institutes of Health. His research interests include evaluating both surgical and non-surgical treatment outcomes in head and neck cancer patients. He is also interested in the complex molecular biology of head and neck cancer, and potential clinical applications of novel targeted cancer therapies. Dr. Mydlarz has contributed to numerous publications, articles, book chapters and lectures. He serves on the editorial board for Oral Oncology and is a journal reviewer for Oral Oncology, Head and Neck, The Laryngoscope and JAMA-Otolaryngology.

Biography/Statement

Dr. Mike Yao is a fellowship trained head and neck surgical oncologist. A graduate of the University of California, Berkeley, Dr. Yao went to Yale University, School of Medicine. He did his residency in Otolaryngology at Stanford University and completed a Head and Neck Surgical Oncology Fellowship at the MD Anderson Cancer Center in Houston, TX.

After completing his fellowship, Dr. Yao joined the faculty at the University of Illinois at Chicago, where he rose to the rank of Associate Professor and served as program director of the residency program. In 2008, he joined the faculty at Stanford University where he specialized in care of patients with melanoma of the head and neck.  In 2011, he moved to Westchester County, NY and was in practice at Westchester Medical Center.  He joined the practice at Mount Sinai in 2014.

Dr. Yao cares for patients with head and neck cancer.  He has a special interest in diseases of the thyroid and parathyroid, melanoma and advanced skin cancers.

Dr. Yao has over 30 publications and been invited to lecture nationally and internationally over 90 times. He has been named to the Best Doctors and America’s Top Doctors numerous times.

Clinical Trials

The Sinai Robotic Surgery Trial in HPV Positive Oropharyngeal Squamous Cell Carcinoma (SCCA) (SIRS TRIAL).  ClinicalTrials.gov Identifier: NCT02072148.  In general, patients with Human Papilloma Virus Positive Oropharyngeal Squamous Cell Carcinoma (HPVOPC) are curable, young and will live for prolonged periods. They are at high risk for long-term toxicity and mortality from therapy. While the long-term consequences of chemotherapy and surgery for head and neck cancer are relatively constrained, high-dose radiotherapy (RT) and chemoradiotherapy (CRT) substantially impact on local tissues and organ function and result in a significant rate of late mortality and morbidity in patients. Studies are now being designed to reduce the impact of RT and CRT for patients.

Patients with intermediate stage HPV positive oropharyngeal cancer will be screened for poor prognostic features and undergo robotic surgery. Patients in whom pathology demonstrates good prognosis features will then be followed without postoperative radiotherapy. Patients with subsequent recurrence will be treated with either surgery and postoperative radiotherapy or postoperative chemoradiotherapy alone. Patients with poor prognostic features (ECS, LVI, PNI) will receive reduced dose radiotherapy or chemoradiotherapy based on pathology. It is expected that over 50% of patients treated with surgery will have had a curative treatment and will avoid radiation therapy entirely and long-term survival will not be changed by withholding radiation therapy to good prognosis patients after surgery. There are exploratory biomarkers of risk of recurrence that will be collected and studied.

There are currently few trials examining the role of de-escalation using surgery alone in intermediate and early T-stage HPV related disease. New surgical techniques have broadened the range of patients capable of achieving a complete resection and the functional outcomes in such patients are outstanding. Furthermore, the sensitivity of HPVOPC to chemotherapy and radiotherapy raise the possibility that delayed or salvage treatment in early stage patients would be highly effective, would result in similar survival outcomes and radiotherapy could be applied to a much smaller population then current standards call for. Looked at from a different perspective, the need for post-operative radiotherapy in this younger, HPV+ and more functional population has not been validated in clinical trials to date.

 

The Quarterback Trial: Reduced Dose Radiotherapy for HPV+ Oropharynx Cancer.  ClinicalTrials.gov Identifier: NCT01706939.  This trial aims to directly compare a reduced radiation dose to the standard of care in HPVOPC for non-inferiority, thus allowing for direct comparison of outcomes between the two groups. The study hypothesis is that LRC and PFS at 3 years for reduced dose CRT are non-inferior to standard dose CRT.

Biography/Statement

Andrew J. Nemechek, MD, FACS is board-certified in otolaryngology-head and neck surgery and is fellowship-trained in advanced head and neck oncologic surgery. Dr. Nemechek serves as the medical director of the head and neck tumor program at Porter Hospital, part of the Centura Health System.  His clinical interests include treating patients tumors the oral cavity and throat structures (larynx and pharynx), thyroid and parathyroid glands, salivary organs, and those with HPV-related malignancies. He also treats patients with advanced cutaneous malignancies, including malignant melanoma. Additionally, he has a strong interest in designing healthcare delivery models and shaping system-wide policies vital to improving healthcare delivery across diverse and at-risk populations.

Dr. Nemechek earned his medical degree from Tulane University School of Medicine, where he also completed training in general surgery and otolaryngology. He completed a combined research and clinical fellowship in advanced head and neck oncologic surgery at the University of Texas MD Anderson Cancer Center. Dr. Nemechek was recently awarded his Master of Science degree in healthcare delivery at the Tuck School of Business and The Dartmouth Institute for Health Policy and Clinical Practice at Dartmouth College.

Biography/Statement

Brian B. Burkey MD, MEd, FACS is currently Vice-chairman and Section Head of the Section of Head and Neck Surgery and Oncology at the Cleveland Clinic Head and Neck Institute.  He also serves as Medical Director of the Center for Consumer Health Information within the Marketing Institute at the Cleveland Clinic, which is a group of 30 people dedicated to the production of multimedia patient health information for all medical needs of patients and their families. Dr. Burkey came to the Cleveland Clinic after almost twenty years at Vanderbilt University Medical Center, rising to Professor of Otolaryngology and Vice-chairman within that department.

Dr. Burkey finished undergraduate studies at the Johns Hopkins University, before obtaining his medical degree at the University Of Virginia School Of Medicine in 1986. He completed otolaryngology residency training at the University Of Michigan Department Of Otolaryngology and a fellowship in microvascular and facial plastic and reconstructive surgery at the Ohio State University, before launching his career at Vanderbilt. He has been an American Board of Otolaryngology diplomate since 1992, and his practice has an emphasis on head and neck oncologic and microvascular reconstructive surgery. He began co-directing the Vanderbilt fellowship in Head and Neck Oncologic and Microvascular Reconstructive Surgery starting in 1992, one of the early fellowships in microvascular surgery, and has trained over 30 fellows during his career, almost all of whom have positions in academic otolaryngology both nationally and internationally. He has continued this fellowship at the Cleveland Clinic, which is accredited by the American Head and Neck Society..

At Vanderbilt, Dr. Burkey served as residency Program Director for 15 years, which spanned three successful site visits. Dr. Burkey completed a seven-year tenure on the Otolaryngology Residency Review Committee of the ACGME, serving two years as Vice-chairman and two years as Chairman of that body. He then served as a consultant with ACGME-International and helped three programs in Singapore gain initial accreditation. He was on the steering committee which founded the Otolaryngology Program Directors Organization (OPDO), and served on the Executive Council and as President of the Society of University Otolaryngologists (SUO). He has also served as a guest examiner and senior examiner of the American Board of Otolaryngology and has been a member of the Board of Governors and numerous educational committees of the American College of Surgeons.

Dr. Burkey has been an active member of the American Head and Neck Society, serving on many committees and is now Secretary of that organization. He serves on the editorial board of multiple journals in the field of otolaryngology, and has lectured extensively on educational and clinical subjects both nationally and internationally. He has authored over 20 book chapters and 100 peer-reviewed articles on head and neck and reconstructive surgery topics. He has been a leader on several cooperative group studies and been a co-principal investigator of NIH-funded research. He continues to mentor residents and fellows and is hoping to continue innovation within all areas of medical education. He completed his Masters degree in Education with an emphasis on the health professions in the summer of 2014. Dr. Burkey is married to Maureen, his wife of over 35 years, and their daughter Rachel Burkey lives and teaches in Erie, Pennsylvania.
